Applicants must meet one of the following criteria and be unable to climb stairs independently:

2. Assessment Requirements
Applicants must obtain an Assistive Device Assessment Report issued by a government-established or authorized assistive device service center, or by a contracted assessment professional from such a unit.

(1) Each household is eligible to apply for one (1) stair climber only.
